How to make changes to your licence
It is important that we are able to contact the Designated Individual (DI) and Licence Holder (LH). If your contact details change please let us know. You can email any changes to licensing@hta.gov.uk.
How to make an application for a licence variation
Applications can be made to vary a licence by either the DI or the LH, with the exception of a Change of Designated Individual (which needs to be completed by proposed Designated Individual and submitted by the Licence Holder / Licence Holder contact). The HTA can also vary a licence by issuing a notice of proposal to the LH and the DI. All applications to vary a licence are subject to evaluation.
Establishments that are licensed under the Human Tissue (Quality and Safety for Human Application) Regulations 2007 (Q&S Regulations) should make an application for a variation using the forms under the Q&S Regulations. All other establishments should make an application using the forms under the Human Tissue Act (the Act).
Change of Designated Individual
If you need to replace the DI due to a change of circumstances (for example change of staff, ill health or long term suspension from duties) the proposed Designated Individual will need to download and complete one of the forms below. This form must be submitted by the Licence Holder / Licence Holder contact.
Change of Licence Holder
If you need to replace the Licence Holder due to a change of circumstances (for example change of staff, ill health or long term suspension from duties) you will need to download and complete one of the forms below.
Change of Corporate Licence Holder
If due to change of circumstances you need to replace the Corporate Licence Holder with another corporate body or replace an individual Licence Holder with a Corporate Licence Holder, you will need to download and complete one of the forms below.

If due to change of circumstances - such as change of staff, retirement, ill health or long term suspension from duties - you may need to replace the Corporate Licence Holder contact. You will need to download and complete one of the forms below.

Changes to licensing arrangements
Should you need to change your licensing arrangements, such as adding a licensable activity to your licence, please contact HTA as soon as possible (email: licensing.enquiries@hta.gov.uk, tel: 020 7269 1900) as a new licence application may be required.
Licence revocation
Should you wish to revoke your licence, you will need to download and complete one of the forms below.
Changes to licensed premises
If you would like to make any substantial changes to licensed premises (for example an extension to the premises) or will be operating out of new or refurbished premises, you will need to download and complete one of the forms below.
