Guidance for Anatomy sector professionals on reporting HTARIs
This guidance summarises what incidents need reporting, how to report them, and what follow-up actions are required

What is a HTARI?
- Anatomy sector HTARIs are serious incidents that may affect the dignity and care of deceased people.
- Designated Individuals are responsible for ensuring the HTA is notified of HTARIs and near-miss HTARIs.
When to Report
- Do not wait for internal investigations to be completed
- Report within 5 working days of
- An incident occurring
- Discovery of an incident
What types of incidents must be reported
Categories include
- Acceptance or release of the wrong body or other human material
- Damage to a body or other human material
- Equipment failure that has a negative impact on a body or other human material
- Incomplete or missing records
- Incorrect disposal or retention of a body or other human material
- Loss of a body or other human material
- Loss of traceability
- Security compromise or breach
- Unauthorised activity involving a body or other human material
- Unplanned disruption of services
Important notes
- Do not include person-identifiable details
- Near-miss incidents must also be reported
- Contact HTA directly for guidance if needed
- All staff working under licence should know reporting procedures
How to report
Initial notification:
- Make sure you have access to the HTA Portal
- Designated Individual or Person Designated to submit through HTA Portal
Follow-up report:
- Submit detailed report through HTA Portal within 2 months
- Please include all relevant documentation
