Report a Serious Adverse Event or Reaction (Human Application sector)
All SAEARs reports should be submitted when the Portal is back online. If you need to report an urgent serious adverse event and reaction please contact Rita Barallon on 07776 460 967.
Report an HTA Reportable Incident (HTARI)
All HTARIs should be submitted when the Portal is back online. In an emergency, please contact Lisa Carter on 07917 158 295
Submit an IA or AA Report
All IA and AA reports should be submitted when the Portal is back online. If your patient requires an emergency living organ donation assessment please call the out of hours number on 0207 269 1991.
