"We are currently reviewing how this happened at Manor Hospital in Walsall, including inspecting next week.  The hospital reported the incident to us as soon as it was discovered and has carried out an internal investigation promptly and in a transparent manner. It is important that the issues are investigated thoroughly, action is taken, and lessons are learned by Walsall and others to avoid similar incidents happening again.”
