Sector-wide compliance assessment for Post Mortem establishments

Following a recent inspection at Nottingham University Hospitals NHS Trust which identified critical failures in the reporting of HTA Reportable Incidents (HTARIs), the HTA is carrying out a sector-wide Evidential Compliance Assessment (ECA).

All Designated Individuals (DIs) at licensed Post Mortem establishments will receive a mandatory declaration on 10 August 2026, which must be completed and returned by 18 September 2026.

DIs are required to review local incident records for the period 1 April 2015 to 30 June 2026 and confirm whether all HTARIs, including near-miss incidents, have been reported to the HTA. Any unreported incidents must then be submitted through the HTA portal by 30 September 2026.

Please note that the results of this assessment will be reported directly to the DHSC Secretary of State, including details of any establishment that does not respond.
