Compliance report licence applications

To apply for a licence you will need to complete a licence application form online. This section provides guidance on completing the application form.

The HTA licences activities in the following five sectors: establishments storing tissue for human application, anatomy, pathology, public display and research. To apply for a licence you will need to complete an online compliance report licence application form.  There is a different application form for each sector. The guidance documents below provide background information about licensing as well as advice on answering the questions in the application form.
