There are five different application forms for: human application, anatomy, pathology, public display and research. An explanation about each application is given below. Human application compliance report licence application
This compliance report is for those who require a licence under the new Human Tissue (Quality and Safety for Human Application) Regulations 2007. A word version of this compliance report is also available for DIs to assess their compliance with the new standards. Please do not apply for a licence using this form.

Anatomy compliance report licence application
This compliance report is for those establishments carrying out anatomical examinations or storing anatomical specimens.

This compliance report is for those establishments carrying out post mortems, storing human bodies or organs, tissues or cells from a deceased person and / or removing relevant material from a deceased person other than in the course of a post mortem.
