Model consent forms

The HTA will be publishing a number of model consent forms on this page. These forms will provide the minimum information required for informed consent.

The model consent forms are not prescriptive due to local variations in practice and may be adapted as necessary for local needs. Model consent forms are part of the consent process and should be supplemented with further discussion and more detailed explanation where necessary.

In Northern Ireland, HSS Trusts and other relevant organisations should use the standardised consent forms agreed with the DHSSPS.

The HTA has also produced other information and guidance which will be needed to comply with the letter and the spirit of the law.

Post mortem consent form

The model post mortem consent form provides a suggested format for Trusts taking consent for post mortem examination of adults under the requirements of the Human Tissue Act 2004. The form may be adapted as users see fit, providing it complies with the HT Act and follows the HTA’s Codes of Practice on Consent and Post mortem examination. It could be used for older paediatric cases, but it is not recommended for babies and obstetric losses.

It is important that staff who take consent for post mortem examination should be properly trained and be aware of the requirements for that particular case.  Although the consent form is important as a record of the consent given, the completion of the form is just one part of the consent process. The full explanation, discussion and time for reflection given to those consenting are equally, if not more, important.

Printing advice

The HTA’s post mortem consent form is produced as a Word file. You can edit the yellow shaded sections to make them appropriate for your Trust. If you are having this form professionally printed, you may wish to consider:

  • Producing this as a 3 page booklet on carbon paper (to enable you to distribute duplicate copies)
  • Alternatively you can take photocopies.

Please take advice from your printing company on how best to achieve this.  

Body donation consent form

The body donation consent form provides anatomy schools with the expected minimum standards to be followed. Anatomy schools may wish to insert additional local information to the form as appropriate.

Donation of allogeneic bone marrow and peripheral blood stem cells for transplantation consent form

For adults and children competent to give consent, when the consultant or senior member of the bone marrow or donor registry team is satisfied that the requirements of the Code of Practice 6 on Donation of allogeneic bone marrow and peripheral blood stem cells (PBSC) for transplantation have been met, they must complete:

1. A consent form that includes a statement by the donor that s/he has received and understood sufficient information to give informed consent.

We are working in collaboration with representatives from the bone marrow community to produce the  model consent form for the donation of allogeneic bone marrow and PBSC for transplantation. Once it has been finalised, it will be available to download from this web page. Please continue to use local consent forms until the HTA issues a model consent form for this purpose.

 AND

2. A declaration by the clinician that they have read and applied the Codes of Practice on the Donation of allogeneic bone marrow and PBSC and on Consent.
