Guidance for completing licence applications

To apply for a licence you will need to complete a compliance report licence application. This section provides guidance on completing the application.

The HTA licences activities in the following five sectors: establishments storing tissue for human application, anatomy, pathology, public display and research. To apply for a licence you will need to complete an online compliance report licence application form.  There is a different application form for each sector. The guidance documents below provide background information about licensing as well as advice on answering the questions in the application form.

Guidance to completing the human application compliance report licence application

This guidance is for those who require a licence under the new Human Tissue (Quality and Safety for Human Application) Regulations 2007. The online application form will be available from 1 July 2007 and should be completed by 5 July 2007.

Guidance to completing the anatomy compliance report licence application 

This guidance relates to the compliance report for those establishments carrying out anatomical examinations or storing anatomical specimens. 

Guidance to completing the pathology compliance report licence application 

This guidance relates to the compliance report for those establishments carrying out post-mortems, storing human bodies or organs, tissues or cells from a deceased person and / or removing relevant material from a deceased person other than in the course of a post-mortem

Guidance to completing the public display compliance report licence application 

This guidance relates to the compliance report for establishments storing bodies or organs, tissues or cells obtained from a deceased person for the purpose of public display. 

Guidance to completing the research compliance report licence application

This guidance relates to the compliance report for establishments storing human organs, tissues and cells for research purposes other than for a specific ethically approved research project.
